Background

Responsible for the planning and commissioning of health services for 46 practices serving over 300,000 patients in West London, this busy CCG was keen to improve their support capabilities to help their practices work better together. In particular, they wanted to help the practices to contribute to and learn from each other’s experiences, to allow more effective and consistent prioritisation and decision making.

Xytal in Action

To gain a comprehensive understanding of the existing culture and approach to team-working and communication, we organised and hosted a series of interventions. Various group-wide interventions provided us with the bigger picture, whilst subsequent meetings and discussions with the practices provided more detailed insights of the practice perspective. Our consultants were now fully aware of the current situation, what ”good looked like” and how the practices could best contribute to realising these outcomes.

Results

Following feedback and a full report from Xytal, the Practices and the CCG saw that their priority was to establish an area-wide prescriptions hub for the management of uncomplicated, repeat prescriptions with more complex cases being sent directly to the relevant GP. With our additional support, the changes required within each practice to properly implement the plan were accomplished and the new hub was a resounding success.
